Rotation Interview Feedback

I am wondering how other sites conduct the interviews for each rotation. We have each intern interview with two departments, and we typically make the determination based on the skills. However, this year I would like to use feedback from the interviewers to help in the placement decision. Does anyone have a form that you provide supervisors for feedback?

Hi Lisa, Hope you’re staying warm up there in Alaska. In the resource guide, I1j, there’s a list of questions for interviewing managers to use as a guide. That might work.
